• No refunds or exchange for cash or other services. Cannot be combined with other promotions. 

• Terms and conditions as set by the hospital. 

• For use at the Personalized Check-up Center, 2nd floor. 

• Excludes specialist doctor’s fees in case of referral. 

• Excludes tests except those specified in the package.
